Welcome to The University of Arizona’s anonymous influenza reporting system. This system was designed to collect basic information from members of the UofA community who may have had flu or flu-like symptoms in the last two weeks. Flu-like symptoms include fever, chest congestion, cough, aches. If you have experienced any of these symptoms, please help us by completing the following questions.

These questions will not ask your name or any other identifying information but will be used to estimate the number of potential flu cases on campus. The survey will take 2-5 minutes to complete.
